Steps to Obtaining a Restricted Driver’s License During Your Suspension:
- Serve mandatory suspension time (the required period before interlock may be installed).
- Complete Information for Modified Driver’s License form.
- Pay the $175 fee.
- Mail form and fee to the address specified on the form, OR submit it in person at the Oklahoma Department of Public Safety office located at 3600 North Martin Luther King Avenue, Oklahoma City, OK 73111
- Make an appointment to have your ignition interlock installed by calling toll-free 800-499-0994.
