- Fill out the Limited License Form 404C and pay the $100 fee.
- Provide proof of SR-22 insurance and proof from the Alcohol Safety Action Program (ASAP) that you have completed your treatment program.
- Make an appointment to have your ignition interlock installed by calling toll-free 800-499-0994.

Steps to Reinstate Your Driver’s License After Suspension:
After serving your suspension or revocation period completely, you may reinstate your driving privileges and apply for a new Alaska driver’s license.

- Complete an Application for Alaska Driver License, Permit or Identification Card (Form 478).
- Pass all required tests (e.g., written, vision, road).
- Submit proof of SR-22 insurance.
